I’m not sure if this is a bug or a hidden mechanic. My ministers are getting disloyal even though I am satisfying the people in their sympathies. Check out my welfare minister here. When i started here groups were much lower. It’s like she harbours some lingering resentment from the previous administrations policies.
It’s a hidden mechanic. Ministers do drop in loyalty over time, all things being equal. The longer they have served in the government, the more they want to retire and genuinely spend time with their family, as well as the more reasons they have built up to disagree with you. Some ministers are inherently less loyal than others, so their loyalty and happiness will thus degrade faster or slower.
