Smoking affects health in general (as I recall). The different policies and values in democracy 3 need to be seen as ‘representative’ rather than exact. For example Asthma is really modelling the effect of a bunch of diseases generally affected by air quality. There is no specific modelling for heart disease, or for lung cancer, or for autism or all manner of other issues, so when the game says asthma, it really refers to a whole group of conditions.
